How Do I Book?

We will agree on a date and time for your first appointment (evaluation).
We will hold your appointment date and time for 48 hours pending payment
Once your payment is received, your appointment is confirmed. See pricing here.
You will receive access to our patient portal where you will be able to easily complete our electronic “new patient intake forms”. No need to print and/or scan. Log in here.
“New patient intake forms” must be filled out at least 24 hours prior to your appointment.

What Do
Evaluations
Consist of?
-Parent interview, review of history
-Clinical observations (parent/child interactions, SLP/child interactions)
-Formal and informal assessment/s
-Based on our findings, we may recommend a frequency of therapy sessions or make other recommendations
-You will receive a full report within 7-10 business days. We will notify you when we upload the report to the patient portal
